Obstructive Sleep Apnea and Pharyngeal Wall Collapsibility

چکیده
Communications to the Editor made of an obstetric patient in this article. As an intensivist who also, coincidentally, happens to deliver pregnant patients, as well as having a strong interest in the care of the obstetrically ill critically ill patient, I would be willing to develop a registry of obstetric patients who develop a critical illness. Included in the data would be the category of critical care diagnosis, such as respiratory failure, eclampsia, cardiac arrest, or miscellaneous (ie, the procedures performed of a critical care nature such as intubation, Swan-Canz catheterization, arterial line, as well as the obstetric procedures such as caesarian-section, dilitation, and curettage, etc.) Of course, demographic data such as age, race, gravida, para, week of pregnancy, size of off-spring, maternal/fetal outcome, all would be germane to this registry. For those readers of Chest who are interested in such a registry, please feel free to forward discharge summaries (without names of patients if desired) with all data incorporated into it. My address is St. Mary’s FP Center, 2243 N Prospect, Milwaukee, WI 53202.

Obstructive sleep apnea of obese adults: pathophysiology and perioperative airway management.
Collapsible pharyngeal airway size is determined by interaction between structural properties of the pharyngeal airway and neural regulation of the pharyngeal dilating muscles. Obesity seems to have two distinct mechanical influences on the pharyngeal airway collapsibility. First, obesity increases soft tissue surrounding the pharyngeal airway within limited maxillomandible enclosure occupying ...
متن کاملMechanical parameters determining pharyngeal collapsibility in patients with sleep apnea.
The relative impact of mechanical factors on pharyngeal patency in patients with obstructive sleep apnea is poorly understood. The present study was designed to evaluate parameters of the "tube law" on pharyngeal pressure-flow relationships and collapsibility in patients with obstructive sleep apnea. We developed a mathematical model that considered the collapsible segment of the pharynx to rep...
متن کاملMRI study of regional variations of pharyngeal wall compliance in cats.
Upper airway compliance indicates the potential of the airway to collapse and is relevant to the pathogenesis of obstructive sleep apnea. We hypothesized that compliance would vary over the rostral-to-caudal extent of the pharyngeal airway. In a paralyzed isolated upper airway preparation in cats, we controlled static upper airway pressure during magnetic resonance imaging (MRI, 0.391-mm resolu...
متن کاملPediatric considerations in sedation for patients with the obstructive sleep apnea syndrome
During sedation and recovery the airway of children with obstructive sleep apnea is vulnerable to collapse. This vulnerability arises from both an inherent collapsibility of the pharyngeal airway in these children and a heightened sensitivity to sedative and anesthetic agents. Pharmacologic and nonpharmacologic support may be required to defend pharyngeal airway patency in children with obstruc...
متن کاملExtraluminal tissue pressure: what does it mean?
In this issue of the Journal of Applied Physiology, Kairaitis et al. (9) examine the control of pharyngeal extraluminal tissue pressure by mandibular advancement and jaw position. Utilizing a novel approach for measuring peripharyngeal tissue pressures, they demonstrated that mandibular advancement decreased tissue pressures anterior and lateral to the airway lumen. Decreases in the lateral per...
متن کاملInvestigation of the link between fluid shift and airway collapsibility as a mechanism for obstructive sleep apnea in congestive heart failure
The increased prevalence of obstructive sleep apnea (OSA) in congestive heart failure (CHF) may be associated with rostral fluid shift. We investigated the effect of overnight rostral fluid shift on pharyngeal collapsibility (Pcrit), pharyngeal caliber (APmean), and apnea-hypopnea index (AHI) in CHF patients. Twenty-three optimally treated systolic CHF patients were studied. Neck circumference ...
